The following items are necessary before your application for admission can be considered.

- Two recent portrait-style photographs, approximately 2" x 2."
- Non-refundable application fee of $30.
- A personal statement (not to exceed 5 pages double-spaced) describing the formative influences on your life, your values, and your sense of religious vocation. We urge you to refer to particular ideas, people, events, family of origin, films, theatre, works of art, church relationship, etc. This statement should express your reasons for seeking seminary education, what you hope to gain from such an education, and what you feel you might contribute to seminary life. - Four personal references. Use this form. Applicant should fill in his/her name and check degree program on the letters of evaluation.
- Applicant should sign the waiver of rights option.
- Applicant should contact his/her references and give them the reference forms from CTS (please do not use family members). We recommend that the applicant supply references with stamped and addressed envelopes. References should mail their completed forms directly to the seminary as indicated on the form.
- The names and addresses of your references should be sent to the seminary on the application form.

Official transcripts of all courses and grades from each college or university you have attended. Transcripts must bear the original seal and signature of the registrar and be mailed directly to the seminary.
